Peri-operative order for kidney transplantation
- Ceftriaxone 2 g IV ก่อนลงมีด
- Methylprednisolone 1 g + 5% D/W 100 ml IV
in 30 min ก่อนลงมีด
- 20% mannitol 250 ml IV drip after vascular
anastomosis and arterial clamp release
- หลังต่อ neovesicoureterotomy ถ้าไม่มี urine
output ให้ประเมิน CVP
- If CVP ≤ 12 cmH2O ให้ NSS 250 ml IV
drip in 15 min x 2 doses
- If CVP > 12 cmH2O ให้ furosemide
250 mg IV drip in 30 min
Postoperative order for kidney transplantation
- Record V/S, please notify if - NPO
- BP <90/60 or >160/90 mmHg - Record V/S, I/O (ml)
- RR <12 or >20/min - BW OD
- PR <60 or >100/min - CBC, UA OD
- BT >38°C - TAC level ทุกวันจันทร์ พุธ ศุกร์ เวลา
7.30 น
- งดท าหัตถการแขนข้างที่มี AVF
- 0.9% NSS 1000 ml IV 100 ml/hr x 2 hrs Medication
then IV rate = urine output/hr – 100 ml/hr - Ceftriaxone 2 g + 5% DW 100 ml IV
Minimum rate 40 ml/hr drip in 30 min OD
- 5% DN/2 1000 ml IV 100 ml/hr - Omeprazole 40 mg IV OD
- Record urine output q 1 hr, please notify if - Prograf…….mg po at 8.00 น และ
urine output < 100 or > 500 ml/hr …….mg at 20.00 น (0.1 mg/kg/day)
- Morphine 3 mg + NSS up to 10 ml IV slowly - Cellcept (250 mg)…….tab po bid pc
push prn q 4-6 hrs หรือ Myfortic (180 mg)…….tab po bid
pc
- DTX q 6 hrs, keep 80-200 mg/dl
- Hct, BUN, Cr, electrolyte, Ca, PO4, Mg,
albumin q 6 hrs
- CXR, EKG
- นัดท า renal transplantation doppler
ultrasound
- นัดท า renal scan
